What you'll be doing:
Provide intensive, community-based case management services to individuals with serious mental illness and/or substance use Maintain standards of timeliness, chart content and face to face contacts with consumers Must be able to multi-task in a fast-paced environment and be proficient in Electronic Health Record, MS Word/Excel and Outlook What you need:
Bachelor's in human services field required; previous case management and substance abuse experience preferred Job Types:
